Minimum Qualifications

Education:

  • Bachelor’s degree in a health or human services field (social work, psychology, nursing, rehabilitation, education, occupational therapy, physical therapy, recreation/recreation therapy, counseling, community mental health, child and family studies, sociology, speech and hearing).

Experience:

  • Three years of case management experience or relevant human services experience required.

Certifications/Licenses:

  • CPR and First Aid certifications (must obtain and maintain).

Work Eligibility: Must be authorized to work in the U.S.

About the Role

The Case Manager supports residents in achieving independence and stable housing by providing case management, service planning, and coordination of care. This role involves developing and implementing individualized service plans, maintaining documentation (ILPs, psychosocials, and progress notes), and collaborating with internal staff and external providers to ensure residents receive appropriate housing, healthcare, and support services. The Case Manager also advocates for residents and works closely with them to establish and achieve their goals.

Primary Responsibilities

& Participate in orientation of new residents by explaining available services.
& Obtain and document assessment information.
& Collaborate with residents to develop and maintain service plans.
& Provide supportive counseling and skill-building.
& Refer and accompany residents to community resources.
& Assist with budgeting and independent living skills.
& Advocate on behalf of residents.
& Maintain accurate case records.
& Attend meetings and trainings.
& Report incidents and comply with policies.
& Obtain and maintain CPR and First Aid certifications.
& Perform other job-related duties as assigned.
